Payment Policies
Visit Information:
Breastfeeding care through LactationMD is provided using a team-based practice model in which both a non-MD IBCLC and Dr. Hall will see dyads jointly for lactation visits. Initial consultations will be scheduled as adjacent appointments for parent and child(ren), typically lasting 75 minutes. Services will be billed directly to insurance using maternal breastfeeding benefits, if available, and medical office visits for parent and child, which both may be subject to co-pays.
For exclusively pumping individuals, initial consultations will be scheduled only for the parent, and then billed to maternal breastfeeding benefits, if available, and medical office visits for parent, subject to co-pay.
Follow up visits will be scheduled jointly between parent and child depending upon the consultation treatment plan. Telemedicine using audio and video technology is available and provided if appropriate.
Referring lactation consultants and physicians/providers will receive a copy of the patient visit, treatment plan, and follow up recommendations.
Insurance Information:
Dr. Hall is an in-network PPO provider with Aetna, Blue Cross/Blue Shield, Cigna, and United Healthcare. She is also an in-network Medicaid provider through Blue Cross Community Health Plans.
Patients with an HMO plan under these insurance companies can contact their primary care provider or OB/GYN for a referral to be seen by Dr. Hall at LactationMD.
Out-of-Network/Self-pay Information:
If we are out-of-network for your insurance, we offer a self-pay rate. Self-pay includes payment by check, credit card, FSA/HSA card, Zelle, or Venmo. An itemized bill, which can be submitted to your insurance company for possible partial reimbursement, will be provided.
A reduced-rate for those who are uninsured and/or provide proof of need (ex. TANF, SNAP, WIC, or unemployment) is available. Please contact us for more information.
Rescheduling/Cancelation Policies:
We request that any appointment rescheduling/cancelations be performed at least 24 hours in advance. Late cancelations/rescheduling and missed appointments for an IBCLC/MD lactation appointment are subject to a $100 fee and for an MD-only visit are subject to a $50 fee.
